Yep, but that form is not so hard to fill out and its‌ just that plus the pro forma tax form.
Your agent should be able to help‍ with this as well.

they charge max 100$ to fill‍ this form. That should not stop you from doing it! If it is does, you⁠ don't need a US company at all!

When i did my research i found cheepest one $500 for both forms.
If anyone‌ has a CPA that does it for $100 please share a link...
