Yep, but that form is not so hard to fill out and its just that plus the pro forma tax form.
Your agent should be able to help with this as well.

they charge max 100$ to fill this form. That should not stop you from doing it! If it is does, you don't need a US company at all!

The cheapest I ever saw was $350

I do but he doesn't cost $100... If someone has a CPA that charges those two form filings $100 I'd love the link to that person..
