I need a personal account. what is required documents and price for that

You need a passport and utility bill as well as a CV and business or personal description. Usually the banks charge a small fee but other than that there is no fee.

If it is something different you ask for then MENTOR GROUP is the only place where you can make such threads.

I didn't find banks offering it to Iranian individuals. Could you give a list

I agree most banks will refuse to open any account for Iranian citizens because of political embargos! You may indeed check mentor group and find your way to EMI's that may be your only possible way to get anything that looks like banking for your needs.
