Nowhere, without legally changing your name. Changing date of birth is not possible (there are fringe cases where someone's DOB has been corrected later in life due to complicated circumstances around their birth).
Anywhere, if you have the right connections and don't mind taking the risk it outside the confines of the law.
IIRC, this used to be quite easy to do in Indonesia and Malaysia where you could easily adopt a Muslim/Arabic name and use that name on your new passport.
It might not help for physical travel since most passports now are biometric, so there's a chance it'll still be possible to connect you to your previous. real identity.

Previously with the old government in Vanuatu - you could legally change your full name, DOB and POB when obtaining Vanuatu citizenship + passport

I know one way, but it involves legalising a birth certificate with different data and having the right connections.

Yes it is possible with the new govt. to do the same, however they are charging an arm and a leg for this now and it is done "under the table" via established connections in new Govt.
I'm talking serious $$ above the regular donation price for Vanuatu passport. In my opinion not worth it, but at the same time - who am I to decide what is worth to a paying and willing client?

Changing your data is the most complicated thing that requires contacts.

Bulgaria is the way. But you need local contacts.
This country is so ridiculously corrupt but there is a high risk that you get busted on renewal of the passport.
However if your goal is to use this passport to get different passports the legal way then that is a good option.

1. Yes but i don't even think that an introduction would give anyone access here to this kind of service, if you don't have established a trusted relationship. It's not like that you just pay money and get what you want. This is like trying to buy a Rolex or Audemars Piguet in a retail store without previous history. But i can tell that if you search in the real estate sector you will find very interesting people that can gain you access to this service.
2. To get a real birth certificate is not really a problem. At least if you know where to search. But there are bunch of things that you need to pay attention to. But probably more worth than buying a passport the illegal way.
