I am about to sign an NDA and it is my first time doing so and really confused about some small and minor stuff. I would like to ask your guidance on the matter. i am adding a part of the contract i need help with numbers.

i need to fill this contract as Individual Freelancers so how do i handle point 1,2,3
also, point 5 and 6. Disclosee and Attention both should be on my name?
