Open bank account 100% online?

I am looking to open a bank account which comes with a visa/mastercard debit card so that I can pay for stuff online (I don't have such card at the moment making online transactions a major PITA). I would be keeping a relatively low balance (few thousand dollars at most). It should be possible to open the account completely remotely as I'm not available to travel. Which bank/service would you recommend?
