Yes that is... I asked the liquidator about that, he didn't answer , and I sent an email to firstchoice if they can send me something and Im still waiting their answer. Screenhots, statements, emails etc are what I have now too

Maybe i'm wrong but if that's the order people will be paid in....we're the last ones. Have you read the notice?"A depositor, creditor and lessee of Choice Bank Limited must complete and submit a Claim Form in order to receive any distribution which may be made by the Liquidator for Choice Bank".It doesn't sound promissing.

So we most of us must fill "Unsecured Prepaid Cardholder"
Can anyone who knows better fill a form as example for others ?
If we don t fill 100 % from first time we will have to wait again and again !
