Is it possible to form a US LLC and appoint nominee director + shareholder?

What service offer this if any?

Just form an LLC in New Mexico. At no point do you provide name of the members and managers to the state. There's nothing special about it that you need to do.

It's the same in a few other states, such as Wyoming and Delaware.

In some states you need to submit an annual report which has to be signed by someone. You can usually just pay your registered agent to submit it for you.

Nominees are not very common in the US, especially for LLCs.

what if someone needs to prove that they are the owners of a US LLC, what document can prove that?
